Free printable blood sugar log
A one-page glucose log for two weeks of readings. Print it, keep it with your meter, and bring it to your next appointment.

What is on the sheet
One landscape US Letter page with a row for each day, enough for two weeks. Each row has:
- Before and after each meal, plus bedtime. Fill in only the times your care team asked you to check, and leave the rest blank.
- Insulin and medications, with doses and times.
- Notes for carbs, exercise, illness, stress or a late meal. These are often what explain an unusual number.
There is a line for your units (mg/dL or mmol/L) so nobody has to guess.
How to use it
- Keep the sheet with your meter and a pen, or on the fridge.
- Write each reading right away, in the column for that time of day.
- Write down every number, including the ones that surprise you.
- Bring the sheets to your appointment, or take a photo of them to send ahead.
Your doctor or diabetes educator will tell you when to check and what range to aim for. This sheet just makes the record easy to read.
Paper or app?
Paper is simple, but averages and patterns by meal take work to add up by hand. My Blood Sugar Journal logs a reading in seconds with meal context, carbs, insulin and medications, shows your trends by meal and time of day, and makes a PDF report for your doctor. It works offline and costs $4.99 once, with no subscription, no account and no ads.
This app is a log, not a medical device. Always follow your doctor's advice.
